Please attach documentation of liability insurance that provides cover for you. (minimum cover of $20M)

Please attach a copy of the organisation's current Certificate of Incorporation.

The organisation must be incorporated under the Association Incorporation Act. Maximum File Upload 2 MB.

Does the organisation currently have a Liquor Licence? If so please upload for our records.
